Self-Insurance Bond (Form 21A)
Description
The Bond form is for use for a Self-Insurer to meet its security requirement. The form language must not be altered and all parts of the form must be completed in its entirety.
Filing Language Requirement:
The Virginia Workers’ Compensation Commission can only accept filings submitted in English.
